From 195 East or West, take the Gano Street exit. From Gano street take a right onto Pitman Street. Follow it and take a left onto Weyland Ave., then take a right onto Seekonk Street. We are directly in back of Whole Foods Market (formerly known as the Bread and Circus Market) on the corner of Butler Street and Seekonk Street.

Cranston Office:

From 95 South take the Reservoir Avenue exit, take a right turn onto Reservoir Ave, then right onto Park Avenue. At the fourth traffic light take a right turn onto Cranston Street. We are in the green building.

From 95 North take a left onto Reservoir Avenue, then right onto Park Avenue. At the fourth traffic light take a right turn onto Cranston Street. We are in the green building.

From Providence: (45-50 minutes)
1. Take Route 95 South to Route 4 South via exit 9 (left exit). Take Route 138 East to Jamestown/Newport bridge. After both bridges, take ramp to end. Go left at light onto Admiral Kalbfus Road (Route 138 East). Follow through several lights and stop signs until you go up a steep hill, go right at light onto Aquidneck Avenue. Family MediCenter is on the right in the Polo Center.

2. Take Route 195 East toward Fall River. Take Route 24 South to Tiverton/Newport, RI. After bridges take exit "Middletown/Newport Beaches". Go right after ramp onto Route 138 South (East Main Road). Travel approximately 8 miles to major intersection (Dunkin Donuts on right) Take left, again toward "Newport Beaches" onto Aquidneck Avenue. Go through next light. Family MediCenter is on right in the Polo Center.

Warwick Office:
215 Tollgate Road, suite 209
From 95 South take Exit 10B, take a right off the exit at the first traffic light, go up the hill, at the stop sign take a right, at the end of the street take a left onto Tollgate Road. We are located across from Kent Hospital.

From 95 North take Exit 10, take a left off the exit, at the second traffic light, take a left onto Tollgate Road. We are located across from Kent Hospital.

Lincoln Office:
2 Wakerobin Drive
From 95 North to 146 (Lincoln/Woonsocket) take Lincoln Mall Exit, at end of the exit turn right onto Rte.116. At the next light take a left into Wendy's Plaza, the large building on the right is the Washington Trauma Center––2 Wakerobin Drive. Sign for Diabetes and Endocrinology will be on the first floor.
